Resumo: Introduction: The transplant is a treatment for many chronic diseases, and aims to save lives and improve the quality of life of transplant patients, computerized monitoring systems can be used to monitor these patients in their homes which leads to cost savings and ensures reaction to emergencies with the necessary urgency. Objective: develop a computerized system for care and monitoring of the transplanted patient. Method: The methodology of this research followed two phases: the first was an integrative review and the second to build a proposal for a computerized system model, the E-Health Transplant. This is an integrative review of research literature, methodological, exploratory and descriptive. Data were collected in the Virtual Health Library, the LILACS databases, MEDLINE and SCIELO and electronic sites. To survey the articles the following keywords were used: "monitoring," "telemonitoring", "transplant", "software", "management", "care", "database" and "interdisciplinary" using Boolean logic through the use of the "AND". The guiding question was: What are the care, clinical aspects and assessment indicators used in national and international literature on the monitoring and care of transplant patients through computerized systems? and adopted as inclusion criteria articles published in Portuguese, English and Spanish with abstracts available in databases for the period between 2004 and 2015; and who met the descriptors eligible for the realization of bibliometrics in journals indexed in the bases chosen for the study. Result: As a result of the first phase of this research, found 549 research after crossing the proposed descriptors, and 50% of the articles found in Medline and 25% lilacs base, 44% were published from 2008 to 2009 and only 16 they met the inclusion criteria and, among these, seven were excluded because they were repeated in two databases. The journal that stood out was AMIA Annual Symposium Proceedings with two publications. The articles were analyzed in detail with the presence of three researchers who extracted the main recommendations for the construction of "E-Health Transplant". The second phase involved the construction of the proposed computerized system that comprises the steps of design, design, creation and system development and system performance with the description of the product with its components. The software of this project is an alpha version, which was built in HTML (Hypertext Markup Language), and Javascript, developed to run on Microsoft Windows platform. The development of the system obeyed the data collection and storage structure that comprises three stages in the process: the SHEET GENERAL PATIENT (divided into six steps: identification, demographic profile, Clinical Profile, Complaints Clinics, Additional Information – medications, vital signs , blood sugar control, diet, invasive devices, bandages, examination and exercise, and Control Scheduling and Monitoring Telephone, EVALUATION, for each of the specialties responsible for the multidisciplinary care and MONITORING and AGENDA, specific area to perform the patient monitoring and control of consultation. Conclusion: The aim was the creation of the model, achieving goals as an organization, standardization and implementation of activities aimed at promotion, recovery and rehabilitation of the patient In addition to foster knowledge and understanding of the factors responsible for potential complications, and organize the necessary conditions to outline the treatment, care and health education in a qualified manner.
